Anhedonia can be a prominent component of a depressive episode. In this paper the author reviews why therapists should focus on anhedonia, defines positivity and presents questionnaires to measure this construct, reviews laboratory and neuroimaging studies, and deficits in recognizing positive affect in others when one is depressed. Treatment efforts to reduce anhedonia are also presented.

Functional impairment and depressive symptoms in older adults: Mitigating effects of hope

The authors point out that mot all older adults with medical conditions become depressed. This research explored the two components of hope (agency and pathways) in a sample of 105 patients in a primary care clinic. The additive factors of agency and pathways was found to be a mitigating factor in level of depression. The clinical importance of helping older clients identify goals and develop ideas on how to achieve them is stressed.

Relaxation intervention for stress reduction among teachers and staff

The author describes a 4-session relaxation training intervention for teachers and staff in a public school setting. The groups lasted 45 minutes each. Compared to a nonparticipating control group positive changes were found due to participation in the intervention in terms of both work stress and life satisfaction. The importance of tailoring a stress management intervention to a particular setting was highlighted.

The impact of the therapeutic alliance on treatment outcome was examined in a sample of children referred to a clinic due to problem behavior. Treatment involved cognitive problem-solving skills training for the child and parent management training for the parents. Results found that the stronger the alliance the more positive the treatment outcomes. Characteristics of the child did contribute to the therapeutic alliance.

Predictors of treatment response to CBT for depression in Parkinson’s Disease

The authors described a 10-week individually administered CBT program for patients with Parkinson’s Disease that targeted their depressive symptoms and also could have included 4 educational sessions for caregivers. Results found enhanced treatment effects when caregivers did participate in the intervention. Further, when the spouse was involved as the caregiver, as opposed to another family member or friend, outcomes were even more positive.

Could exposure to everyday green spaces help treat children’s ADHD

Could children’s play environments be related to symptoms of ADHD? The authors explored ADHD symptoms and the play settings of 421 children. The authors found that children playing in built settings (both indoor and outdoors) had more severe symptoms than children playing in outdoor green settings. Income and gender had no impact on these results. The authors suggest it important to optimize children’s time in green settings.

Mindfulness-based stress reduction is a growing intervention that has evolved out of the mindfulness movement. In this study participants completed weekly assessments of both mindfulness and perceived stress. The authors found quick changes in some mindfulness skills and this subsequently had a positive impact on reductions of perceived stress by participants in the 8-week program.

Nonsuicidal self-injury is arising problem area for adolescents. It is not only seen in adolescents identified as clients but also in the general adolescent community. At least 1/3 of the sample reported engaging in NSSI at least once, and one-sixth engage in the behavior at least five times. It was more common in females than males. The authors describe these results in the context of an experiential avoidance model.

Chronic pain involves a biological component but there is an emotional component to the pain experience. Chronic pain patients frequently find there way into the offices of practicing mental health clinicians. The authors discuss adapting to chronic pain from a resilience model, including recoverability, sustainability and growth. They discuss this model from both an intrapersonal domain and interpersonal domain. They also discuss the role of stable indicators and those that may be modifiable. The authors advocate for incorporating social components into the assessment and treatment of chronic pain.

Videoconferencing psychotherapy: A systematic review

Providing psychotherapy services by videoconferencing is gaining in popularity. There are many logistic reasons why this may be a preferred mode of service delivery both in the public sector, but in private practice as well. However, in order to justify this alternative modality research must document that it is an effective way to deliver services. The authors review the literature regarding providing psychotherapy via videoconferencing along ten separate questions. The research suggests that this is an effective mode of delivering psychotherapy services but that larger scale clinical studies are still needed.

Childhood trauma can be complex and children can easily experience comorbid diagnoses besides PTSD. In this paper the authors address the question of whether childhood interpersonal trauma results in an inter-related set of symptoms, examines the biological correlates of symptoms commonly occurring in maltreated children, the effects of childhood interpersonal trauma exposure on treatment, and whether symptoms can be accounted for by any one diagnosis. The authors suggests a new way to define or conceptualize childhood victimization or interpersonal trauma as a better way to understand this clinical problem area.

The authors examine the multidimensional dimensions of mindfulness and how they relate to both physical and psychological health (negative affect, existential well-being, social well-being). Results found a relationship between mindfulness and cardiovascular health. In addition, more mindful individuals had lower levels of social inhibition and less negative affect.

A certain percentage of women are at risk for postpartum depression. The present study examined the role of perceived social support from their partner during pregnancy on later psychological adjustment after the baby was born. Lower levels of emotional distress were associated with higher levels of perceived social support from their partner during pregnancy. Interestingly this was also associated with lower levels of infant distress. The importance of assessing quality of the parental relationship during pregnancy is stressed by the authors and there may be a role for mental health intervention to improve postpartum adjustment in couples low in perceived social support.

There is a high relapse rate following treatment for alcohol abuse. Therefore it is important to identify treatment approaches that have more success than others. The authors found drink refusal training to be associated with better drinking outcomes right after treatment and at one-year follow-up. The importance of monitoring a client’s self-efficacy in being able to reduce their drinking behavior is noted from this research.

Couples can come to therapy with differing goals. In this study the authors examined therapy goals prior to entering counseling to determine if they were predictive of treatment outcomes. For those whose goal was primarily to improve their relationship had better outcomes when compared with those that wanted to clarify the nature of their relationship. The importance of understanding client goals is stressed by the authors, as well as understanding that goals may shift as therapy progresses.

Little attention has been paid in the literature to the development of OCD after age 65. In this paper the authors explore issues of assessment, comorbidity, prevalence, types of symptoms and treatment approaches with this population. Two detailed case examples are provided to the reader. Initial impressions in this understudied group suggest that CBT may be an appropriate approach to treat this problem area.

Being able to empathize with others can be beneficial but it can also cause emotional distress. The authors present data on 3 studies in the development of a questionnaire that measures vicarious distress. Three factors emerged in the questionnaire: Distress, Avoidance and Support. As this is a frequent (15%) occurrence in couples developing interventions that targets this population is important. In this article the authors present a case study of one couple who were treated for this using Acceptance and Commitment Therapy. The treatment (which lasted 20 sessions) approach is described and includes discussion of creative hopelessness, values clarification, mindfulness and compassionate acceptance, cognitive defusion, exposure exercises, and maintenance. In this paper the authors review literature related to dissemination and adoption of effective treatments across health care settings. This paper describes an IOP for dual diagnosis patients that is provided in the context of a private practice. A brief description of the program is provided along with the outcome measurement process. A significant number of adolescents self-harm. However, these are not always with the intent of suicide. The authors examined differences between nonsuicidal self-harm and suicidal adolescents. Differences emerged between the groups based on sex, age of onset of self-harm, and method of self-harm. Clients were randomly assigned to a 30-minute therapeutic assessment session based on a manual that is described in the paper. Those with nonsuicidal self-harm had a more positive therapeutic response than the suicidal group. The need to assess for suicidal intent in self-harming adolescents is emphasized.

Many couples will never think about seeking out marital therapy. In order to overcome traditional obstacles to seeking out treatments the authors describe an innovative program, The Marriage Check-Up. Similar in concept to an annual physical check-up the authors describe an assessment and motivational feedback session for couples. The authors found that couples with mild to moderate marital distress would seek out such an assessment. They point out that in order to reach couples they must self-perceive their relationship as healthy and not in distress.
This approach can be adopted by entrepreneurial private practitioners who are interested in expanding the marital therapy part of their practice.

In this study the authors examined the role that seeing blood plays in a sample og 64 college students with a history of skin cutting. More than half of the sample thought it important to see blood, and nearly one fourth looked at the blood for more than 5 minutes. Tension reduction and helping the person feel calm were the most frequent functions of seeing the blood. Those who viewed blood as important engaged in more frequent cutting behavior than for those who did not view this as important. Clinical implications of these data are discussed.

Bariatric surgery in adults is becoming a more common and accepted treatment for morbid obesity. Less is known about the outcomes of bariatric surgery in morbidly obese adolescents. The authors followed 37 adolescents who underwent bariatric surgery 4 months following surgery. Significant positive changes were found on measures of depression, anxiety, and self-concept. However, a small subset showed difficulties in their mental health adjustment suggesting the need for close psychological follow-up of this patient population after weight loss surgery.

The authors compared a health education intervention with a CBT, family-focused intervention for family members of patients with bipolar disorder. After participating in a 12-15 session intervention family members found themselves to be less depressed.

The authors examined the effects of a mindfulness cognitive set and the impact on worry. People who are more mindful worry less than those who are less mindful. Mindfulness components of less judgment and acting with awareness were related to less worry. An unexpected finding was that higher levels of observing one’s thoughts may in some conditions increase worry. The authors suggest that openness to experience may be a key factor in reducing worry.

Brief Strategic Family Therapy vs. treatment as usual: Results of a multisite randomized trial for substance abusing adolescents

Treating adolescent substance abusing clients is a difficult task. In this paper the authors compared a Brief Strategic Family Therapy approach with treatment as usual in 8 community settings. Because of the multisite approach a large sample (N=480) of adolescents were included in the study. While there was a minimal amount of difference between the treatment approaches on self-reported drug use, the family therapy approach did improve retention in treatment and also improved family functioning. Clinical implications of the data are presented.

The authors present a systematic review of 8 studies that target Internet addiction. There are significant limitations in this area of research. Body Dysmorphic Disorder is a difficult problem area to treat. The authors tested a manualized and modularized approach with 12 adults in individual sessions. The authors describe the components of the approach including psychoeducation and case formulation, cognitive restructuring, exposure and ritual prevention, mindfulness/perceptual retraining, advanced cognitive restructuring, relapse prevention, and modules that focused on skin picking and hair plucking, muscularity and weight/shape, and cosmetic treatment. As a result of the intervention body dysmorphic test scores were reduced to the nonclinical range and depression became subclinical as well.

The authors tested a SmartPhone DBT application (DBT Coach) for individuals with diagnoses of borderline personality disorder and substance abuse disorder over a 10-14 day period. Clients were encouraged to use the technology as often as they needed during this time period. A description of the technology is provided by the authors. Clients found the application to be effective and engaging, to have a positive impact on reducing emotional intensity and urge to use substances, and to have a reduction in depression and general emotional distress.

A systematic review of psychosocial interventions for suicidal adolescents.

The authors present a meta-analysis of outcomes 17 studies that had targeted interventions for adolescents who were high-risk for suicide. A Table is presented including information on the study design, sample/demographic information, program description and duration, and the setting in which the intervention took place. Results were not encouraging regarding current interventions for reducing future suicidal ideation and self-harming behaviors as compared to treatment as usual. Booster sessions following initial treatment is one recommendation considered by the authors.

Thje authors developed a 14-item scale and present psychometric data to support its reliability and validity. The authors advocate for routine screening of firefighters for occupational stress given the brevity and adequacy of the instrument They suggest such screening may help with early identification of firefighters at risk for developing serious psychological and substance abuse problems.

The authors present discussion on research related to core beliefs, imagery, and core beliefs in eating disorders, preparing the patient for imagery work and presenting a rationale to patients. A case description is also presented including transcripts of the session and useful tips for the clinician in doing this type of work.

Cognitive therapy for insomnia has been well tested with adult populations but less so for adolescents. Three adolescents participated in a 7-week (one hour per week) CBT-manual-based program and were followed for 3 months following the end of treatment. A description of the seven sessions is provided. Results found a crecuction in insomnia severity and this finding was maintained at 3 months follow-up.

Children with autism spectrum disorders often have comorbid behavioral problems, including difficulty regulating their emotions. Children participated in 9 one hour group sessions focusing on skills training and parents participated in a psychoeducational group. Results found those children in the skills training group exhibited less emotional outbursts following the training and when outbursts did occur they tended to be shorter in duration. The authors suggest that CBT training may be an alternative to the use of pharmacotherapy with these children.

In this study the authors explored the concept of deep acting versus service acting and the roles of introversion vs. extraversion. Deep acting led to emotional responses that led to service providers exceeding customer expectation, thus resulting in higher financial rewards. However, surface acting was also found to be effective but this was moderated by extraversion.

In this study the authors examined the effects of 3 20-minute writing sessions on stressful or gay-related trauma event in a sample of gay men. Results found positive effects on acceptance of sexual orientation. This intervention was most powerful for those with the lowest levels of perceived social support. Implications of these results for those working with gay men in psychotherapy are presented.

A significant number of college students have ADD/ADHD. This disorder may interfere with their academic success. A group of students with ADHD and without ADHD were asked to estimate how long it would take to complete a task. Those with ADHD were poorer predictors of how long it took to complete the task compared to those without ADHD. Implications of this data for working with college students with ADHD for improving academic success are discussed.

Patients often have a difficult time communicating clearly with their health care providers. Patients were trained to communicate more clearly with their physicians using a structured written form. Those patients who participated in the intervention experienced a more positive office visit with their physician and physicians rated a stronger partnership with these patients. As mental health providers are more integrated into the health care system one role that may be played is teaching patients how to communicate better with their doctors thereby improving their health care experience.

It has long been thought that cruelty in animals by children has been a predictor of later antisocial behavior. In teenagers in Switzerland 8% of girls and 17% of boys self-reported intentionally maltreating an animal at least one time. Such behavior was predictive of later delinquent behavior, but most predictive of violent delinquent behavior.

Integrated behavioral intervention to improve HIV/AIDS treatment adherence and reduce HIV transmission

It is important that individuals with HIV/AIDS comply with their antiretroviral regimen, and also not engage in behavior high-risk to transmit their disease. Unfortunately not all those in need do so. The authors tested a behavioral intervention that targeted improved compliance and a reduction in engaging in unprotected sex. Positive results were found and maintained at follow-up.

A study of self-injurious and suicidal behavior in a veteran population

The authors examined suicidal behaviors and intent in a sample of veterans. Contrary to being an impulsive behavior the majority of suicide attempts were planned out. There were differences between patients and staff in ratings of intention to die, with patients ratings being a better predictor. Exposure to combat was found to be a significant risk factor for completed suicide.

Traditionally couples have not done a good job in integrating a newborn into the family. Because of this John Gottman and his associates developed an intervention to help improve this difficult transition. In this paper the authors examined the effectiveness of the intervention on later play activities within the family. Results found that couples participating in the intervention felt less competition with each other when playing with their new child. Results also suggest a beneficial effect of completing this workshop during pregnancy, rather than after the child is born.

The authors present a meta-analysis of 44 studies examining personality characteristics of problem gamblers. The effects of PTSD in service members may not only have an impact on the soldier but also may impact those in their family. The authors explored the presence of stress related symptoms and PTSD-type symptoms, as well as general psychological distress in a sample of spouses of veterans with diagnosed PTSD. They found a general level of distress in the sample, wit ha subset experience PTSD-like symptoms. However, not all of the distress could be attributed to their spouses’ military experiences and difficulties. Implications of these data for interventions with these spouses are discussed.

In this paper Johnson and Barnett examine this from both an ethical and practical perspective. They discuss issues related to countertransference reactions, common reactions to a life threatening illness (emotional distress, denial and fear) reasons why a gravely ill psychologist might choose to continue to practice, life-threatening illness and the ethics of professional competence, and problems associated with self-monitoring professional competence (self-assessment bias). The authors also present recommendations for the practicing mental health professional, professional colleagues, and for credentialing and regulatory boards.

Less is known about treating PTSD in adolescents compared to treating this disorder in adults. The authors developed an 8-week program of modules that focused on psycoeducation about PTSD, relaxation training, the teaching of anxiety management skills, and cognitive restructuring. Those completing (75%) the program received a reduction in symptoms of depression and anxiety and these findings were maintained at follow-up.

The use of stimulant medication by college students is on the rise. However, little is known about the effects of taking such medications in a nonprescribed manner. Epidemiologically it appears that use of these stimulants is commonplace among college students. The history and pharmacology of these medicines, the effects on cognitive performance, whether they enhance learning and memory, the impact on cognitive control, and the impact on other executive functions are all reviewed by the authors.

In this study the authors had one group write about their first heart attack and compared outcomes with a group of heart attack survivors who did not do this type of writing. Results found that the group who did the writing had fewer medical appointments, were more involved in the rehabilitation process and had better health outcomes compared to the control group.

The wars in the Middle East have resulted in a significance increase in traumatic brain injuries for our soldiers. It is important to understand what treatment approaches work best for these returning soldiers, as well others who have experienced a TBI. The authors conducted a meta-analysis of 27 studies examining the role of severity of injury and treatment outcome. There was a medium-sized effect between severity of injury and outcomes measured at one year. Interactions of variables and clinical implications of these data are discussed.

In this paper the authors examined patient self-reports in treatment of past violent crimes including murder, physical assault, and sexual assault, for which they had not been arrested. One in almost 7 psychotherapists have had the experience of patients telling them they had “gotten away with murder” and higher numbers had heard about physical and sexual assaults. Ethical implications of these data are discussed, as well as the effect that such self-disclosure might have on the psychotherapy relationship.

The authors examined personal characteristics and interpersonal predictors of participation in a parenting group for disruptive children and behavior change in the child. The authors describe a 6-session program to reduce problem interactions between child and parent. Results found that depression in the parent was predictive of the child’s behavior change. Marital status and conflict between the parents was predictive of the number of sessions attended. Implications of these results for conducting parenting groups and improving participation levels are discussed.

The authors review research related to perinatal health and relationship satisfaction, parenting efficacy, and pregnancy responsibility. Results of the study found parenting efficacy to be related to relationship satisfaction, and the more concordance between mother and father the higher levels of relationship satisfaction and lower levels of depression.

A sample of 63 children and adolescents underwent a prolonged exposure program for the treatment of PTSD designed by Edna Foa and her associates. Half of the clients experienced a rapid decrease in their PTSD and depressive symptoms as a result of the treatment. These gains were maintained at follow-up. Treatment alternatives other than the standard protocol need to be developed for those who do not experience this rapid gain.

The authors describe the primary ingredients of habit reversal treatment and the efficacy of the approach. Their meta-analysis included over 500 subjects across 18 studies. The authors concluded that large positive effect sizes were found when this method was applied across tics, habit disorders, and stuttering. Early-arriving girls experienced the highest levels of social anxiety compared to on-time girls, and early and on-time boys. Social anxiety and puberty timing was not an issue for boys. In this study college students reviewed photographs of clinician’s office and the students impressions about the counselor. Mental health professionals are often called upon to treat unusual problem areas. The case report summarizes a behavioral intervention for a patient who was a hoarder and who also experienced dementia. The authors present the functional assessment of the behavior, a reinforcer assessment, and describe the interventions with this patient. A nompharmacological intervention focused on “sorting” appeared to be an effective approach to treating this targeted problem area with this patient.

However, little is known how these philosophies may impact treatment outcome. In a sample of clients undergoing outpatient substance abuse treatment the authors measured substance use, criminal activity, arrests, and employment for those treated in women only and mixed gender groups. Those treated in the women only group had less substance use.

Most cognitive therapy interventions have been developed for adults. However, there is ag rowing literature to see if these concepts can also apply to the treatment of children and adolescents, who theoretically have less developed cognitive abilities than adults. The authors describe the results of a 5-15 session pilot program which achieved results comparable to adults in reducing anxiety levels in these younger clients. Most clients achieved remission of their symptoms and none of the clients uses medication.to achieve these positive outcomes.

Many people wonder why victims stay in violent relationships. In this study messages to self and others were explored in samples of both male and female victims of intimate partner violence. Few differences were found between males and females regarding reason for staying. The most common internal reason for staying was, "I would have been a failure if I left the relationship." The most common message communicated to others was, "My children needed both parents." The author present discussion regarding the clinical implications of these data.

As there may be significant secondary gain (e.g., financial benefits) a certain portion of these clients may not be truthful during their evaluation. Social anxiety is a complex clinical problem. Through their research the authors were able to identify and measure three aspects of negative self-portrayal. These include social competence, physical appearance, and signs of anxiety. In using this scale specific aspects of negative self-portrayal can be identified so clinicians can better target interventions for the individual client.
